Step 1 :  DOWNLOAD AND PRINT THE SIGNUP KIT.

"Right Click" each number 1-3 and "save as"  1     2     3     

Download to someplace you know how to find - your desktop or documents usually.

If you don't have an image editor that PRINTS, here's a free one you can install [click]  IRFANVIEW

1.  2257.jpg This is the Model Release, proof of legal age and your right to work in the US.

2.  w9.jpg  This is the document we use to report your earnings to the IRS.

3. Naked Performer agreement 2010.doc  This is a Microsoft WORD document. If you need a reader/viewer, go here.

Step 2 : FILL THEM BY HAND OUT USING THE INSTRUCTIONS & EXAMPLES BELOW.

 1. 2257.jpg   Use blue ink pen please! Use the example and the instructions below.

NOTE: For all dates please use 00-00-0000 format. Example: June 30, 2009 would look like 6/30/2009.

Section A - print your FULL legal name EXACTLY as it appears on your PHOTO ID including middle name or initial. EXACTLY.
Section B - list the issuing state, type of ID and ID NUMBER. For example: " California Driver License # 123456789"
Section C - List your "stage name" or the name you will use as performer on webcam.
Models sworn statement: Print your full legal name again just as you did before, print today's date and sign.

Producer's / Photographer's Sworn Statement: Print today's date and the city and state you live and perform in.  We do the rest.

 It should look like the example below when you are done:

I know some of the examples are typed, they want YOU to fill them out in your own handwriting.

This is a W-9 form from Internal Revenue Service.

Print your name (as it appears on your ID)
Check off individual/Sole Proprietor unless you actually already own and operate a Corp, LLC or partnership.
Fill in address, city, state & zip
Part I : Fill in your Taxpayer ID Number (Social Security Number)
Part II: Sign and date.

 On Page 1 at the top:

1. Print today's date.
2. Next is your full name as it appears on your ID.
3. Next is your date of birth
4. Sign your name at the bottom so we know you read the page.

 On Page 2:

1. There is nothing to fill out on this page. Just sign your name at the bottom so we know you read the page.

 On Page 3

1. In the section "Termination" print your email address in the "if to model" section.
2. Sign, print your full name and date at the very bottom at "Model Signature" & "Model Printed Name"

 It should look like the images below when you are done. Please do these by hand . They want YOURS to be handwritten.

 STEP 3 :  Scan all 5 pages as pictures (JPG) back into your computer in color.

They have to be scanned in color so they can see you did them in blue ink.

If you do not have a scanner, you can visit a Kinko's or other type of pack and ship type shop or office depot,  staples etc. If you need help locating someone or using your scanner, let us know.

OR..........

 If that is not an option and you have a digital camera and just a little patience, you can take PICTURES of the pages and send them back  - please do not resize them or otherwise process them please. They have to be readable.

STEP 4 : SCAN ID's and TAKE A PICTURE HOLDING YOUR ID's NEXT TO YOUR FACE.

 1. We need a full color scan or good quality READABLE PHOTOGRAPH of your ID's all by themselves. 

 Get a sheet of white paper and sign your name in blue ink below the ID's and then scan them or if you do not have a scanner, photograph them with a digital camera.

 2. Then we need a CLOSE UP color picture holding these ID's against the side of your your face.

The text on the ID's must be clearly legible in the scans and photographs.

 If you have to squint or turn your head sideways in order to read them - it's a DO-OVER.

CHECKLIST: REVIEW YOUR WORK.

1. HANDWRITTEN. Completed and signed copy of 2257. Use blue ink.
2. HANDWRITTEN. Completed and signed copy of Performer agreement. Sign all 3 pages. Use blue ink.
3. HANDWRITTEN. Completed and signed copy of w9. Use blue ink.
4. COLOR SCAN/photo of your ID's. Sign your name in blue ink below them. You can not use an expired ID.
5. COLOR photo of your face with your ID's held up to/next to your face.

ALL ID text and numbers must be readable in scans and photographs.

STEP 5 : EMAIL IT ALL BACK TO webmaster@giirlzinc.com
